If your doctor decides you need a medical certificate, script, or referral, you will be provided those documents at the end of the consultation. If this is a phone consultation your script can be sent to your mobile phone directly as an e-script.
Medical certificates and referrals can be emailed to your personal email address at your request. Original documents will be available to pick up from the reception, with proof of identity. If someone else is collecting those documents on your behalf, a request letter with your signature is required. Non-collected documents will be securely disposed after sixty days.
